Who should consider undergoing a fertility assessment?

What does a fertility assessment entail?

Generally, a fertility assessment involves the following steps:
Medical History Gynaecology specialist Dr Ma Li begins the process with a thorough assessment of your medical history, which includes general health, menstrual cycle, past pregnancies or miscarriages, past surgeries as well as prior fertility or ovarian-related treatments.
Pelvic Examination She then proceeds to perform a physical examination by checking the structure of your pelvis. Doing so will enable Dr Ma Li to detect any abnormalities with the physical structure of the pelvis.
Blood Test A blood test is done to evaluate your hormone levels. Depending on Dr Ma Li’s initial findings, these hormones can include:
  • Anti-mullerian hormone (AMH)
  • Follicle-stimulating hormone (FSH)
  • Luteinising hormone (LH)
  • Prolactin levels
  • Thyroid function (TSH)
Additionally, a blood test may also be performed for ovarian reserve testing, which will enable Dr Ma Li to count the number of antra follicles (eggs) in the ovaries.
Imaging Tests A hysterosalpingography (HSG) is an imaging test that enables Dr Ma Li to evaluate the openness (patency) of your fallopian tubes and determine if blockages are present. Meanwhile, an ultrasound scan will help Dr Ma Li assess your ovarian reserves, uterine health as well as confirm the presence of cysts, fibroids or any other abnormalities.

FAQs

Frequently asked questions

It’s recommended to undergo a fertility assessment:

  • For women under 35: After 12 months of trying to conceive without success.
  • For women over 35: After 6 months of trying to conceive without success.
  • If you have a known medical condition that could affect fertility, such as PCOS, endometriosis, or a history of irregular periods, it’s advisable to get assessed earlier.
Fertility assessments are generally done once when you are actively trying to conceive and haven’t been successful within the typical time frame. However, if your situation changes, such as difficulties conceiving after delivering your first child, or are diagnosed with a medical condition, a repeat assessment might be needed to check progress or changes in fertility.

The difference between a fertility assessment and a pre-conceptive screening is:

  • Fertility assessment: Focuses on evaluating your reproductive health, including ovarian reserve, sperm health, hormone levels, and any physical barriers to conception.
  • Pre-conceptive screening: Assesses overall health before pregnancy, including genetic carrier screening, vaccinations, and any conditions that could affect pregnancy, like diabetes or high blood pressure.
In many cases, negative fertility results, such as hormonal imbalances or lifestyle-related issues, can be managed or improved through medical interventions and lifestyle changes. However, certain factors, like age-related decline in ovarian reserve, may not be fully reversible but can still be managed with treatments like IVF or donor eggs and sperm.
No, a negative result does not necessarily mean you cannot have a biological child. It may indicate reduced fertility or challenges in conceiving, but many people with negative results are still able to conceive naturally or with fertility treatments such as medication, IVF, or other assisted reproductive technologies.

Yes, depending on the cause of infertility, treatment options include:

  • Assisted reproductive technologies (ART): such as in vitro fertilization (IVF), intrauterine insemination (IUI) or the use of donor eggs or sperm.
  • Medications: to regulate ovulation.
  • Surgical procedures: to remove blockages in the fallopian tubes or correct uterine abnormalities.
  • Lifestyle changes: improving diet, exercise and reducing stress.

Yes, lifestyle changes that can improve your chances of fertility include:

  • Avoiding exposure to environmental toxins, like chemicals and pesticides.
